The following form should be filled out by a Prichard resident if they would like to receive a proclamation from the Mayor.
To complete the document accurately and efficiently, please provide as much information as possible.
If you are requesting a proclamation, please cut and paste a copy of a previous document or include enough information to draft a new version.
Please submit form no later than 14 days prior to date needed.
Please note that all criteria must be met to grant your request, and all requests for proclamations or commendations are subject to approval.
